Never got Slayer Form?

As in the title, I’ve never got the slayer transform ability after the maze. This also happened during my last playthrough, but after a while I suddenly got it. No luck this time though.

I’m tempted to add the ability via EEKeeper, but when I looked up “Slayer” in the spell database, several versions came up (I have the relevant component in the Ascension mod installed btw). If anyone has the ability working correctly, could you look it up what the code for it in EEKeeper?
