Quest related items are not being generated in my game preventing me from completing quests.

    The dialogue action that initiates the teleportation cutscene has Brage give the sword to you - a GiveItem() action. It should go directly into the protagonist's inventory, and actually before you jump to the temple. If the inventory is full, another item (normally whatever's in the first slot) will be dropped on the ground back where Brage came from.
    The version of Brage that's in the temple is actually a separate creature, and he doesn't have the sword at all; if you somehow had him trigger that dialogue, he would be incapable of giving you the sword.

    The kobolds that drop the vials are a pair of melee attackers on the second level of the mines, just before you descend to the third level. The vials are not flagged as critical items, so exotic deaths like freezing the kobolds are capable of destroying the vials. It's an ordinary drop, so you of course have to pick it up. And if you killed those two near other kobolds, that may require scrolling through the quick loot because there are just too many junk items on the ground.
    Nearly all of the enemies in the Nashkel mines, including that pair, are pre-placed. The only creatures that ever spawn in are the group chasing Miner Beldin and the reinforcements Mulahey calls for.
