Extremely Minor issue in chapter 3 at the Mage's dungeon.
I have been playing NWN since it came out, and I remember a particular thing that is happening that isn't any longer in chapter 3.
When you use the portal to enter the mage's dungeon and get to the south-western section, where the elementals spawn. In my game, fire elementals spawn, I don't know if anyone else has that issue. I have played Neverwinter Nights since it came out and that area used to spawn water elementals. I checked in the Diamond edition and fire elementals do spawn in Diamond. I wonder if there is anyway to get the water elementals to spawn again, since that area has the summoning pool to summon a water elemental. Again, this is a minor issue, but it is a bit annoying to go into an area expecting water elementals and getting fire elemental. It also breaks immersion for me since, if both portals only spawn fire elementals, where did all the water elementals come from?
Chapter 2 has a minor thing too. in the section where Baram's quarters entrance is mobs used to spawn and if you went to where they were spawning at you found a secret door that, if entered would bring you to a house in the section where the city gates are located. I don't think they will ever get fixed, I am just reminiscing and hopefully they one day will get back to the way it way it was, of course minus the intellect devourer in chapter one at the bottom of the prison not dropping it's brain after getting killed.

I just played through Luskan last week. The breach in the wall near where Kurth's dudes spawn is still there. It's not secret—more so since the secret doors were introduced in the next expansion. For Baram's side, you can go in the sewers in various locations, and in there you can get to his backdoor entrance. Again, not really secret. I do remember thinking they changed or removed things from my first time playing with the first updates back in like 2003 and 4, but I'm pretty sure I imagined those things now. (Also it's been so long that I don't even remember details, just that I looked for things and could not find them, or things didn't happen like I remembered.)
For the intellect devourer I might have had that happen once out of about ten times. The on death script for it progresses the quest and creates the brain. If the brain doesn't get dropped, the quest should also not update.
The main small issue I still have that was introduced with EE, is you cannot activate the door levers in The Pits area from both sides anymore. Instead your character tries and fails to run through the wall to get to the other side of the lever. If you don't have Open Lock skill, you cannot fight the dogs.
