What happened to my journal entries?

I've still got the ones made by myself, but all the the quest ones except "Important Events" are gone. I've checked all chapters and both the Quests and Done Quests tabs. They're gone.

I didn't change anything and I've never used any mods, and the journal entries are gone from all of my saved games.

    OK, I read that traveling to a new map fixes it, and that worked fine.
