I don't remember well that part, and I'm not even near that chapter with my assassin character, but does the plane of hell we visit in BG2 have any relation to Bhaal? Why is there demons (tanar'ri) in that Hell (isn't hell supposed to be "Baator"?)? I know Bhaal is lawful evil, does the alignment determine what kind of fiends are in service of a god?
Also, if evil person die he goes to hell or abyss, am I right? And it is possible to travel to either plane through planar travel? So isn't it possible for the damned to actually get out somehow?

If I remember correctly, if a mortal dies but either does not worship a particular god, or has not fully impressed their chosen evil god, they will be sent to either the abyss or hell. I have a feeling this differentiates depending on the mortals alignment. Soul trading is also common and many mortals will often trade their soul to either a demon or devil for great power. These souls can then be traded between devils and demons, essentially transferring the deceased mortal from one plane to the next repeatedly.
This can happen to good aligned characters who worship good aligned gods, if they have committed certain atrocities that betray that particular gods dogma. For example: A fallen paladin of Torm who committed evil prior to death, may be sent to hell.
Something to also remember is that the planes of hell and the abyss in the Forgotten Realms are just that - planes of existence. Planar travelers can, and will often travel such dangerous planes looking for treasure or power.
In relation to your question on whether or not you can escape hell or the abyss - Well this is a difficult one to answer. Mortal's who die have their soul's taken. Once their soul is taken, they would require it to be freed, which is not an easy feat. Until this happens they are trapped to whatever being holds them, and essentially in whatever plane that beings resides. Planar travelers have not 'died' and thus their soul belongs to no particular deity, demon, devil or plane of existence. They travel because they are free to do so. Deceased mortals are not.
In regards to BG2, it could be said that CHARNAME is delivered to hell due to the fact that Bhaal no longer exists in Gehenna. CHARNAME's soul is corrupted and thus has no logical destination in the afterlife no matter who they worship. They were dragged there by Irenicus and thus had no right to be there anyway.
Personally I consider this an oversight by the original BG writers. You're very correct, Demon's are not supposed to be in hell - they would be hunted and killed by all devils. The writers probably did this to make it easier for players who are not overly knowledgeable about the Forgotten Realms. There can be a lot of confusion when you try to explain all the different planes of existence, demons, devils, hell and the abyss.
The way I try to look at it is: The place you go to at the end of BG isn't actually hell as is known in FR lore. It is probably a reserved plane of existence specifically formed for the children of Bhaal. This would explain the inclusion of Sarevok, Irenicus and a multitude of extra-planar beings which would be able to travel to and from this plane. It would also explain the BG2 ending in which Irenicus is viciously attacked by a large number of 'Slayers' which I always assumed were other children of Bhaal who had been slain and had been corrupted fully by their blood now that they were in this 'hell'.
