I don't understand. If you have the weapon proficiency, you can use it.
Single weapon style:Have a single weapon equipped, no shield, no second weapon in off-hand. You will receive +1 ac and bonus to critical hits.
Two weapon style: Any character can place a second weapon to their left hand to dual wield, however without these weapon style you will get soo many penalties to your attack roll you won't be able to hit anything. The more points you have here, the less penalties you will receive. Rangers like Minsc start with two pips in this one, so they can dual wield (use two weapons in each hand) quite effectively.
Sword and shield:any character can use a shield (save for mages) but with this style the character will receive further bonuses against missile attacks.
Two handed style:With a two handed melee weapon, like a staff, spear, halberd or two handed sword, you will receive damage, speed and critical hit bonuses.
To add to @lunar's post. This is not a special ability type thing which you must select and then attack with. It's a persistent attribute which effects the math on your to hit rolls and AC depending on how your character is equipped. If you have a proficiency in e.g. single weapon, every time you attack while only wielding a single weapon you receive the bonuses.
The Weapon(sword) icon is simply Attack. The Shield icon a Defend/Hold command in which the character will hold his/her ground and attack anything that comes in range. Neither has anything to do with the abilities you choose for your character.
To "use" Dual Wielding you simply put a weapon in your mainhand slot and another weapon in the shield slot on the Inventory screen. The paperdoll should now show your character holding two weapons.

Ah ha, you put the weapon in the Shield slot!! That's the part I was missing and couldn't find anywhere.
Sorry to all , I was so lame the way I was asking my question.
No worries @buckbeach. Don't take my liking your above post the wrong way, you're not lame. If you didn't ask then we wouldn't have a chance to show off our awsome game knowledge and get those "likes" "agrees" and "insightfuls"
Hey hey, some "likes" "agrees" and "insightfuls"! Man, hope i'm not coming off desperate for "likes" "agrees" and "insightfuls". Like this post if you think it's desperate. Insightful this post if you like this post. Agree this post if you disagree with my pandering for "likes" "agrees" and "insightfuls"

as many mentioned Two-Weapon Style or Dual Wield is only "valid" on Classes who can at least put two ** in Two-Weapon Style. otherwise you get so many penalty to THAC0 (hitting stuff !) that you basically just flail those weapons around to no effect.
Namely Ranger (gets 2 for free) and Ranger/Cleric Fighter and every Fighter/X Dual and Multiclass Thief(only Swashbuckler) Bard (only Blades) Paladins

That's not necessarily true. Solo mages for instance can make good use of dual-wielding daggers to attack enemies who are disabled (by sleep, hold, emotion etc).
