What makes you a Fallen Paladin in BG1 & BG2

I was just wondering if there is a good list somewhere about what choices and/or quests will cause your main character Paladin to become a "Fallen Paladin" in either or both Baldur's Gate games? Any help would be very much appreciated. Thanks!

    I don't recall any specific quests outside of an event in the Windspear Hills (which never seemed to actually make me fall). Otherwise, it happens if you drop below reputation 6 or kill an innocent (whatever that means).

    I seem to recall that siding with Bodhi instead of the Shadows Thieves in BG2 will cause you to fall. This happened to me the first time I played BG2. I was on my paladin & didn't trust those shifty Shadow Thieves, so when I got another option, I explored it. Meeting the shadowy figures in the Graveyard District, it didn't occur to me that they might be vampires, so I accepted their seemingly generous offer - only to lose my precious paladinhood. Ouch.
