Some sort of... double death?

Kagain was slain in a mighty struggle against a dastardly foe, but when he was raised from the dead, he came back sort of... stunned, or petrified. Waiting or resting did not help, nor did casting Stone to Flesh: he's just there, but he's completely frozen and unresponsive and his inventory is unavailable, just like someone would be when Held - but he's also greyish-black, as if petrified.

Please report what he got into, area/enemy name. Any clue what effect persisted over death.

That bunch of rude mercenaries in the basilisk area west of Beregost and temple, where you get Shar-Teel from. As much as I gathered, he just got killed by loss of hit points, though whether it was a sword or a spell or whatnot that finally proved his undoing I can't remember.
