Many unique items have a + designation associated with them, all weapons and some rings for example. I'm not sure what the rules are but unique alone does not seem to decide the description.

The rule is Name +Enchantment Bonus (if not 0). An item's "actual name" is used when available, so, for example, a Quarterstaff +3 has the tooltip "Aule's Staff +3." This change was decided through a public poll a few months ago; about three-quarters wanted the unique names and many also wanted the enchantment bonuses displayed as a quick way to gauge an item's power.

Yeah, the poll just supports the unique name, not unique +x.

The item's name is Shield of the Falling Stars. Shield of the Falling Stars +1 implies that somewhere out there is a Shield of Falling Stars +0/+2/+3/etc. We really only should be adding +x to the name if it's needed to distinguish the item, i.e. Short Sword +1.
