Missing Spell Effects in AR0512 (Fuernebol's Room, Kiel's Room)

When the greater doppelganger attacks you in Fuernebol's room (x=1200 y=900), it is supposed to trigger a spell effect (Cloudkill if I remember correctly), but it doesn't.
Similarly, only one of the three greater doppelgangers that attack you in Kiel's room (x=1900 y=700) trigger its spell effects. I believe all three are supposed to do so.
