The item stipulates that you would get -4 in death saves and +3 in the rest but what actually happens is that you get +4 in death saves and -3 in the rest.

the AC bonus works fine

    For saving throws, less is better. It wasn't wise to remove the 'penalty' and 'bonus' words.

    The description convention for saving throw bonuses is the same as that for Armor Class and THAC0 bonuses: + is better and - is worse, because we're talking about the bonus and not the actual value (if we were, there would be no +/-, just the number).

    The text indicates that the wearer becomes more susceptible to poisons (penalty to saves vs. Death) as part of the price for being harder to target (bonuses to all other saves). This is working correctly. You see this on the Record screen as +4 and -3 because, as @Avenger_teambg said, lower saving throws are better. Kind of like how you equip shields with a +1 AC bonus and see your Armor Class go down.
