One thing that was always a potential matter of discombobulation during multiplayer games was leaving Pause open to everyone. If two players try to Pause the game simultaneously, one can potentially keep unpausing the other player's Pause in a flurry of attempted Pauses... while monsters have at them. This tweak would allow a setting in the Hotkey Options to set a key for Unpause. This way whoever hits the Space Bar (or whatever's been designated) will indeed Pause the game, and Unpausing will be a much more deliberate function. Obviously it only works if all players set their consoles similarly, but still.
I assume it would have to have at least the scripting needed to change the Pause key setting from a reversible toggle to a latched one when the Unpause is enabled.

By saying have a seperate unpause key, are you asking for the option for space bar to be used for pause only? So if several players press pause simultaneously, the game will pause and nothing more. That I imagine would be the best way for it to work.
No matter what system they elect to fix it, it does need to be fixed.
The current system does solve this somewhat (from my experiences of paying single player "multiplayer" games) by not allowing the game to be unpaused for a couple of seconds, but this is really annoying when you realise that you paused unnecessarily. I'm far too trigger happy with pause
