Any items needed to keep?

Spoilers

The item inciting this post is the girdle of masculinity/femininity. Do I need this for anything?

Are there any other items such as gems that I need to save?

I know a skull for melicamp.

Any others?

Use it on Edwin for the lawls. Otherwise sell it :)

@claudius: You don't need that girdle for anything important. You might find it amusing to use it, but that's all.

You don't need any gems for anything in BG1 except on one occasion in Durlag's Tower (where the relevant gem and its use are both found close together). Any gems you find anywhere else, just sell them for gold. (Although when we get to BG2, there are gems which actually serve a purpose.) Edit: oh yes (thanks ajwz), I was forgetting that there's a use for a sphene gem in BG city, although it's a minor use and you can always buy one back if you don't have one on you at the time.

Yes, a skull for Melicamp, but you can usually find one when you need one by killing a skellie right outside High Hedge, no need to store one specially.

Other than that ... well, obviously there are various pieces of good equipment which you'll want to keep. There are a few quests where you have to find objects and do something with them, but generally you're told plainly what you're seeking, so those are obvious too.

There's really nothing you need to keep which isn't made fairly obvious, except this: Keep the pantaloons! Always keep the pantaloons!

You don't need the girdle for anything...unless you want to romance all three characters at once.
All you need to do is briefly become a woman (and be good-aligned) while Rasaad is in your party.

Perdue's Short Sword is needed for an optional quest, but a lot of people either miss it or accidentally sell it because it looks exactly like a regular short sword.

If you bring an ankheg shell to Taerom in Beregost and refuse to sell it to him (refuse, don't haggle), he will offer to make you a set of armor from it. Ankheg shells are easy to obtain, but you need to bring one to him within ten days after you get it, I think.

In the city of Baldur's Gate, you will need an angel skin ring and a ruby ring for two optional quests. The ruby ring is obtainable as part of the quest, but it doesn't have a special name, so you might accidentally sell it.

You might want to import the Golden Pantaloons into BG2 for an easter egg.

It can be useful to get your hands on an angel skin ring before ch5, and also a sphene gem.

Have the golden pantaloons and the chainmail +3 in your inventory for your final save, as these items will follow you to BG2. Some others can as well (including the claw of Kazgaroth) but they replace other powerful default gear like the helm of Balduran.

I always keep an angel skin ring and a sphene gem on me (especially with the new gem bag) for 2 quests in Baldur's Gate city.

Winter wolf pelts are worth keeping for the shopkeeper in Nashkel.

Nester's Dagger drops off an ankheg to behind Tenya's house in Wyrm's crossing, it's for a quest in the city too.

Corpses from the city sewers are used in a quest in a house just south of Sorcerous Sundries.

Give the belt to Jaheira since she acts all bossy and manly.
