I keep my rep middle of the road. The populace shouldn't fear me like a bandit, but they need to know I'm not to be trifled with. My Evil PCs are (usually) socially adept, so I'll feign nice in some instances, but in others I let people know they need to check themselves before they wreck themselves. If I figure out an NPC has some kind of sweet loot, I will generally figure out a way to discreetly kill them.
I'd like to play Evil, but I simply can not stand the raging BS that is the Reputation system.
^ This. I only seldom play evil, and when I do, then it is smart evil. Basically, I started out with my Archer being good, but his relationship with Viconia corrupted him, and after the inclusion of Dorna in the party...which he also got lustily attached to (strangely enough, Vikky didn't mind him rutting about with a she-orc or with the wild witch). Then Baeloth found his way into the party, and things got a bit more evil. Rasaad must be wearing out his prayer beads! His feeble attempts to cure my wicked ways are...amusing. This is why I keep him in the party!

Rasaad is mumbling again, ferverently fumbling with his prayer beads. Vikky sways over to him, crooning about her Goddess "Shar is a goddess of determination and conviction".
I completely ignore the reputation meter and just take whatever path I think will net me largest reward. If a bunch of gullible saps think I'm a goody-goody because of it, great, easier to backstab them later or pin the blame for evil acts on someone else if necessary. Who you gonna trust more...some sap off the street, or a "courageous adventuring hero", celebrated in several towns for their good deeds for which they were LAVISHLY rewarded with coin, magical items, adoration from the ladies and the like.
Usually lower mid range like ~5-8 I like to play evil and kill who I want if whim or loot dictates it, However i usually try to keep my rep just high enough to not be hammpered by legions of flaming fists at every turn as well as be dealt unrealisitic prices from merchants. I play tactical evil, more how i would envision a successful evil group acting, random violence and public mass murder dont benifit anyone they simply hender your progress to your true goal, besides once char name has ascended the throne of Bhaal theyll have all the time they like to harass and masscre the innocents.
I pit fight my PC against the NPCs I encounter along the way (as I usually play solo or as a custom party when evil). Those that survive in hand-to-hand combat are rewarded with one of the games many cursed items.
Even when I have a Thief class, I kill Algernon for his cloak.
I slaughter the Sirines in the Beregost Temple and leave the scene a desecrated vigil of Helm's ineptitude.
I flood the mines with the slaves inside.
In my most recent playthrough I was already in Baldur's Gate when I remembered about the Xvart village. I took a weeks vacation to make sure I killed every last man, woman, and child.
I only fight waves of Flaming Fist when I'm returning from Candlekeep and I'm strong enough to deal with them.
The mine is probably my favorite thing I've decided to do with my latest Evil playthrough. I chose to do nothing with the floodgate, with my CHARNAME intending to take over the mine for himself after he's dealt with everybody who knows about it.
I play evil the way the game seems to want me to play. That is, be a complete ****hole, do horrible things for the hell of it, and slaughter people for no apparent reason! HAHAH!
I often play evil characters, but usually choose the neutral response in dialogue. The evil choices make you sound pathetic and childish.
Not a D&D game, but the most fun I've ever had playing an evil character was in Knights of the Old Republic. Some of the stuff you could do was so monstrous, it even made me flinch.

Anyone who has played Neverwinter Nights 2: Mask of the Betrayer, that game had THE BEST evil playthrough ever. Thank the gods that the writers realized people want to be evil without being morons. You can literally EAT A GOD! in that game.
I think most of my neutral characters end up going a little insane-evil trying to constantly save their own hides My evil characters just start out that way hah

Sadly, a lot of games have difficulty getting past this, much like they have difficulty getting past violence as the only solution to every problem. Then again, while we're used to massacres, more "petty" or personal evil might put people off a bit more.
