(See the last page of the Known Bugs sticky on this forum for the experienced problem).
The crux:
Leaving the tower after Kiel's helmet spawns on the floor (at MINIMUM the moment you enter the second lower floor of Durlag's Tower, but possibly the moment you ENTER Durlag's tower), can and will cause the helmet to despawn, locking out your ability to progress through Durlag's tower without using the Developer's Console (or something like iFunBox/Shadowkeeper) to force spawn the helm in the player's inventory.
Note also that the recommendations in the linked thread above WILL NOT work for the iPad because the Developer's Console is NOT accessible through the iPad interface. If you experience this bug you have only 2 options:
1) Reload a saved game prior to leaving Durlag's Tower. This could mean reloading a saved game BEFORE ENTERING Durlag's tower if the timer for the floor items decaying starts the moment you enter the tower.
2) Offload your Saved Game to a Windows/Mac Copy of the game and use the Developer's console to Create Kiel's Helm (HELM14). This should cause the trigger effect immediately and allow you to continue the game. NOTE: In this case, iFunbox/Shadowkeeper may NOT fix the issue. Because Shadowkeeper is simply adding the item to your inventory whatever the trigger effect is may not be triggered. In other words you may log in and load your game to having the helm but still no way to progress.
Recommended Action:
Once you've entered Durlag's Tower, progress AT LEAST to the second lower level and through the Throne Room to pick up the helm BEFORE leaving the tower to sell items. Use a lootable container to store any leftover items you cannot carry with you so they don't disappear as well.
Recovering:
If you've done this and are not able to progress you can use the following item codes in the console of the PC or MAC game to get going again:
* Kiel's Helm - HELM14
* Durlag's Goblet - MISC98
