Possible bug (v1.2) - multiple save vs. spell rolls against a single spell

See the attached screenshot below - characters are making multiple saves vs a single spell / effect. This is the second time I noticed this. First time was when earlier a ghast was attacking my party members, those who were affected by hold person effect displayed "Held" message twice (i.e. "Khalid-Held" immediately followed by another "Khalid-Held").

Also:
1) Combat log indicates Jaheira made the save - yet she WAS affected by fear along with Imoen and Rassad:
2) My character (party leader) WASN'T affected by fear despite the fear effect icon appearing on his portrait.
