Hexxat mod request (spoiler-free)

So, a simple request for people who know what they're doing. At some point, with Hexxat, you get A Certain Cloak. Now, it's handy to remember whether or not you have that cloak equipped. What I'd like is a mod that replaces Hexxat's small portrait (and possibly her large one) with the concept art (you see it every time you start an SoA game) when she has the cloak on, and restores her normal portrait when she takes it off. Is this even possible? I know Edwin's portrait gets swapped around (with BGTweaks) at a certain quest point; I'm wondering if the same thing can be done for putting on or taking off an item.
