It's possible to miss the trigger that starts Chapter 7
It seems to be possible to miss (or otherwise not set off) the proximity trigger that starts Chaper 7 after emerging from the Candlekeep Catacombs.

After talking with Diarmid it seems to work every time no matter what dialog path I take... but if I sneak around Diarmid outside without talking to him (or if I simply kill him with one backstab and exit), there's no Chap 7 narration. Same on multiple tests. (Mind you, this test was from porting directly from the game start to AR2619, so I'm not sure if the trigger is based on already having completed Chap 6.) The Chap 7 narration works thematically whether you've gotten info from Diarmid or not since he's not mentioned in it and everything that is mentioned in it is already known to the player, so interacting with Diarmid doesn't appear essential to a Chap 7 trigger.
The other issue is whether Diarmid's dialogue really is essential- either to the main story or to triggering Chap 7. One thing that's very useful is his mention of Sarevok being back at the Iron Throne headquarters- though even that is buried in dialog choices, not the first thing he says, so that info can be missed even if you intentionally speak with him. It's very difficult to contrive a forced dialog that would bypass the player's Inviso or stealth without being heavy-handed. In vBG1 Officer Vai force-talks to an Inviso'd party member- looks odd (but doesn't happen in Build0629, so...)
It does seem more likely than not to end without the Chap 7 trigger in vBG1 though, whether you've spoken to Diarmid or not, so things have improved.
Tested this in two ways:
CTRL+Y'd Diarmid before he could speak to me and then I zoned out.
Used CTRL+J to bypass him and exit the are.
In both cases Chapter 7 was triggered.
Confirmed Not Fixed for CLUA purposes.
Wearing BOOT01 and using RING05 I just slipped right past Diarmid thrice to no Chap 7 trigger...
However, I'd just CLUA'd to AR5506 from Candlekeep start. Not sure if you did the same, @Tanthalas.

If I use CLUA to advance chapters to 6 first, then it works every time.
