[DUPE] - Slayer form problem

My problem is that after gaining slayer form, i used it against the demon in the gnome village in underdark and after that, that form just disappeared, no matter how many time i sleep, how many time i load, that form is just not here anymore.
Is there a way to get it back or is it wished like it?

    Same problem here. I sleep like a vampire over centuries but it didn't return in my ability list.
