Hello :)

Is there a counter on how many times I can say no to Clara about going to Dragomirs tomb?
What triggers her instant death when she's imported to TOB and can it be prevented?
And finally is it possible to put a permanent silence effect on a companion?

    No, but it will be too late after Chapter 3.

    This is by design. She has no content in ToB and is meant to leave the party if the player keeps putting off her quest in SoA.

    Not without modifying the game.
