After you get to Ulgoth's Beard with Soultaker, some mad cultists steal it from you to revive Aec'Letec, a Tanar'ri.
But if you pause the game and drop the dagger before they talk to you, the dialog triggers anyway and you can keep the dagger even if it should be destroyed.
Easy fix: add a block to the BALDUR.BCS to make it delete this weapon if the cultists took it away from you.
