Portraits turning black

I'm really sorry if this has an obvious answer or if there is already another post about it but I couldn't find anything about it. Sometimes when a member of the party gets hit the character portrait get filled up in red, as is intended i assume. But other times it just gets black instead of red. How can I fix this?

if the character is still alive but gets hurt, the picture turns red to indicate % health missing. if it turns grey, then they have 'died' but can be resurrected at a Temple (costs gold) or a mid/high-level divine caster may have the spells to do it themselves (Raise Dead/Resurrection).

you may want to look into ways to better protect your companions (spells, equipment, easier quests, different tactics, check for traps and disarm them...)
