It's magic asylum. Irenicus simply can't kill character.
P.s. There should be a way to make him flee without freeing prisoners, insta death is a bad designers choice.

1) Load attached save (this is attached to the ticket in redmine)
2) Have Anomen cast Death Ward on Abdel
3) Open the door to the soul removing room
4) Walk to 1000.1130
5) Talk to Irenicus (e,e)
Observed
Imoen and Anomen will die, however Charname (Abdel) lives. The dialogue with Irenicus will continue to loop until Death Ward is no longer active.
Expected
Charname should also die.
