BG2:EE: Achievements broken? (possible spoilers)

Hi. I'm not sure if the achievements are simply not functioning correctly for me, or if they just have very specific criteria that the achievement text doesn't mention, but I've been having trouble with a number of achievements. Some I've been able to get, but for example, ones like:

- In the Image of Balduran -- have all four pieces, equipped them all simultaneously, nothing....

- Planehopper -- went through this quest line to save him from the extra-planar prison, though no achievement afterwards....

There are a few others that I've thought I should be getting that don't seem to appear, and it may be me and not the game, so I was just wondering if others have been encountering similar issues?
