How Does the Spawn System Work?

I'm noticing significantly less enemies in the game. With a party of six at level 1 in the area east of the starting area (first notable FR character encounter) I cleared the fog of war and apart from the quest related ogre, found exactly zero enemies. Then a single Xvart spawned for me to kill on my way to exit the map.
I now have a party of 6 at level 4 and am still not seeing as many enemies as in the original game. As I level up can this be expected to change as described in the difficulty settings?. Set to insane difficulty.

It seems (at the moment) that the 'insane' difficulty level is based around re-spawns and ambushes (which do seem to recognize party # and level) but I could have easily cleared over 8 maps so far with the above average original enemies on the map...resulting in the game not giving me as much xp and being significantly easier, even on the insane level.
So to get the hard experience do I grind levels with huge difficulty spike ambushes to get the game to throw harder enemies at me?. Otherwise I'm worried that pretty much all maps won't have any placed enemies in them.
I was impressed with what I saw in the video detailing difficulty setting in SOD, but from the developer response in a thread it seems that isn't retroactive in BG1 & 2...?
