At What Leval Does Your Party Get The Extra Enemies?

As I have mentioned in many threads (sorry, just enthusiastic) I'm on a run using SCS for the first time.
Because of that and with all the talk of increased difficulty, for the first time in a long time I have built a strong full party.
But I'm finding the extra enemies I seemed to always get with a small party that was obviously higher levels, are not turning up.
No Lich in the Shadow Temple, no big mobs of trolls in D'Arniss Keep, no Adamantine Golums in Planer Sphere, ect.
So are they based on average party level, Charname level or something else?

Also the normal game mechanic which decides what level of spawns you get depends on average character XP. I don't remember precisely what the thresholds are, but I have a feeling they get tougher at 1.2m XP, 1.5m XP and then possibly the hardest opponents (liches etc) are somewhere around 2m XP per character.
