Sorry, this is clearly in the wrong section but I see no way of creating a discussion there.
During the Paladin stronghold questline, you will have to protect Tyrianna in the house from assassins. If you click on the stairs immediately after battle ends, you will go up the stairs at the same time as the cutscene for Tyrianna coming downstairs triggers (where she complains about the blood on the carpet). The result is you go upstairs before that happens, another Tyrianna spawns and both of them just tell you to "go back downstairs" and the quest cannot be completed.
Very easily replicated. I did it 3 times in a row.
