"Level Up" Icon Not Appearing on Portraits?

Greetings all,

Has anyone else had the issue of the gold "level up" plus sign not appearing over their characters' portraits? The level up sound effect plays, and a notification appears in the dialogue box, but no icon appears.

This makes it easy to miss when someone has leveled up. Note, the other icons (for acid arrow, fear, et al) work just fine. I'm playing the original campaign with Siege of Dragonspear installed, v. 2.5.17.0.

The only mod I've installed is to restore the game's original movies and chapter/dream sequence pictures.
