Scaling a Cliff? Difficulty Check or Die?

I wonder what the best way to do this would be... I have a cliff section - where I'd like for the PC's to have to roll a dexterity check (to successfully climb) or an unsuccessful check (fall to their death) ...
I am thinking maybe a trigger that gives the PC a dialogue to attempt to scale the cliff or not
and wondering how this could be scripted... thanks!

It implemented movement skills by using placeables. When the PC clicks on a movement placeable the first time it gives them an idea of how tough the movement (climb/swim/jump/etc) will be. When they click on it again, it performs the action. If they fail, damage can be applied - even enough to kill someone who fails the check, if they badly miss the DC.
