Spells from items don’t always work?

I have two items in my inventory that I quite like: a ioun stone that allows me to cast an unlimited number of True Sight, and a rod that allows to cast a limited number of Ruby Ray of Reversal. But for both items, sometimes it’d take a couple of tries before the spells stick. Fyi Ruby Ray of Reversal doesn’t allow a saving throw. So what could be wrong here?
