Missing some items, due to mod?

So I'm playing through BG2, and I notice certain items are not where they are supposed to be.
Belm for one, wasn't in the chest in the Ogre's tower. The Horn of Valhalla is nowhere to be found. And I just finished the Unseeing eye quest, and did not find Dragon's bane. Might have missed others. Some stores are also missing things, could only find one store that was selling a scroll case, though there should be several.
I'm playing with quite a few mods, see list below. Most obvious candidate for moving the weapons is Item Revisions (V4 beta 10), but I can't find anything in the documentation (incomplete as that may be). Anyone know what could cause it, and where they could be?

    Probably IR. I know the Horn of Valhalla is moved by one of the mods I use to the temple in the de'Arnise keep stronghold. Figure it's Item Revisions, though I wouldn't rule out SCS.

    Yeah seems likely. I had no idea the horn was moved there, so you're only ever going to see it if you use that stronghold.
    Still really like this mod, but I wish the documentation was a bit better.

    Pretty sure it is IR indeed. I found Belm, had only been moved a few yards, into the hands of Ithafeer. Don't really mind the change, just finding certain items is going to be a chore.
