Dual Wield Bug. Secondary weapon keep disappear from the slot

.... it just vanish. I buy 3 longsword and they disappeared from the off hand slot. Im puzzled.

    I am having this exact same issue. My dual wield Kensai has replaced 3 longswords in the last couple hours of gameplay because the offhand keeps diappearing. It's not getting dropped into inventory... it's just vanishing. Making it difficulty to enjoy my character choice :(

    Where those magical weapons? Is it possible they broke?
