How do you fill plot major holes ?

I lost permanently both Dynaher and Minsc in one miserable fight in SoD. They were both hit for critical damage and died without possibility to resurrect them.

But I know that in BG2 they reappear, or rather only Minsc as Dynaher is declared dead. Either way, how do I justify this plot hole ? Any role-playing ideas ?

I do know that there is an element of mystical resurrection involved because I remember one time I similarly had lost Jaheira in BG1 and she reappeared in SoD. Before she got disintegrated to pieces by a lightning I gave her all three tomes of Wisom in BG1 and after I found her in SoD she still had + 3 Wisdom on her character stats suggesting that she must have been resurrected after her death. But I have no clues from fantasy role-playing perspective how that is possible or how to justify it as some mystical phenomenon.

    I lost Khalid too. It gets better right. He died in that Boreyesker Bridge fight against the crusaders. He wasn't even in my party. I just found him dead on the bridge. I wouldn't have recognized his body if it was not for the items he was carrying as a pack mule from BG1. All my items lying there on the bridge on his corpse. Jaheir couldn't even resurrect him with Harper's Call. Must have died critically I guess.

    I was taking care of the crusader brute Barghest with my bounty hunter, just laying traps around him after I charmed him to follow me outside the castle with Algernon's Cloak. Apparently Khalid decided to launch an attack at the same time. I didn't tell him to do so because I was taking care of them one by one. See what happens when Khalid doesn't listen to me ? He gets himself killed.
