Spellhold bug??

I recently regained my freedom in Spellhold but when I wanted to go back into the asylum to fight Irenicus I encountered a strange bug. Irenicus appeared out of nowhere and repeated his initial dialogue and sent me back to the play games with Bodhi all over again. Now I am stuck and cannot get out. Help?

    Do you have a save you can zip and attach please @sylvanus‌

    & to be clear, this is at the stage where you have concluded the sanity test and should return to free the inmates?
