Chaotic Neutral becomes Lawful Neutral.
True Neutral stays the same.
99% sure. I'll test that later and edit this.
Edit: Lawful Neutral CHARNAME changes to Chaotic Neutral.
True Netural Jaheira "changes" to True Neutral.
So yeah it swaps Chaotic with Lawful and vice versa, Good with Evil and vice versa, and True and Neutral stay the same.
The helm also doesn't seem to make Rangers or Paladins fallen so, if you want to have a Chaotic Evil Paladin... you kind of can.

15k if you kill him at the top, 15k if you kill him at the bottom. And you can do it both times for 30k.

Just saw your post. I'm wondering if you got glitched because I KNOW I killed him on the top floor a few weeks ago and got xp. I'm going to try this later to see if I still get xp.

You are right, it didn't give me xp. I'm either dreaming or something changed, honestly the first one is pretty likely.
Still going to add this to a bug report just incase.

You can't recast the spell. I suspect that if you wore the helm on a Fighter / Mage BEFORE you cast the spell the first time. The resulting familiar would be based off your new helmet changed alignment. I'm guessing this because equipment restrictions are effected by the helmet.
For example my evil Fighter / Mage was able to wield Drizits scimitar Twinkle (Good Only) and wear the Robe of the Evil Arch Magi (Evil Only) by putting on the robe, then the helmet of opposite alignement, then Twinkle.
@moopy: I just killed him today and got no XP.
No, I had an all good party that I wanted Dorn in because I haddn't used him yet.
Got him the helm of opposite alignment, raised the parties reputation to 20, and he stayed no problems.
From a role playing perspective I see it as CHARAME tricking Dorn into wearing the helm.
Lower resistances with Greater Malison and Doom and then hit him with a wand of paralzation.
Edit: I normally run with 3 arcane casters so I have one arcane caster hit him with Malison and some type of Cleric hit him with doom, and the second they both hit I have the other two mages that haven't used their turn yet ready, each of them with a wand of paralzation.
Yeah, it wasn't a very sophisticated strategy, but I'm not that great at the game.
That would make sense from a realistic standpoint, because however it happened, you didn't "technically" kill him I guess, since he appears later.
