How does summoning work? Pronounce some syllables and poof a fire elemental is wrested out from its plane to do the bidding of its conjurer? If the summoned creature dies in this plane, does it cease to exist in its original plane?
What creatures can be summoned? Can prime materials such as humans/elves, halfings, etc. conjured by other creatures like demons? Imagine LG paladins conjured and dominated by CE demons.

Anyone on the outer planes is fair game, though. Solars, paladins, devils and demons.
Most creatures- especially fiends- can only die in their home plane unless they're very low ranked. So when you kill a balor on the Prime/in Faerun, it just reforms (in a year and a day, usually) in the Abyss. This is why the deal with the Imprisoned One in ToB had everyone in such a tizzy: if he was killed, he'd just reform in the Abyss in a year as good as ever.
While PCs can't be summoned to the outer planes they could be affected by monster summoning type spells which summon existing monsters from the prime material that do permanently die if killed in service.
As I see it, summoning - or using any magic - is part using the right words, down to inflection, and proper gesturing. Being able to channel the energies in the first place is another thing.
