so from what I gather of the souls AI they steal party health and give it to the bladed skeletons which ups their maximum HP which is the cause of the near death bladed skeletons taking somewhere around 500 points of damage before dieing which may be an intended feature as i never got that far in the original.
It works fine for all the souls expect for the Severed Soul on the first floor he seems to steal party health and give it back to the party which increases your maximum HP.
I have included a save I was testing the bug with just go down the stairs and fight the battle downstairs it may not happen every time so a couple of gos may be in order.

Looks like it happens when a bladed skeleton is brought to badly injured, and then not killed before the next Shadow Pact (or at least on the difficulty level I tried, it might vary).
I was low on health so I used Turn Undead to make them switch sides and tried to heal my cleric with the Pact. I didn't even realize something was wrong until I got 309 HP. It seems to be permanent as well.
