The death of the lich before Phreya causes a dialogue glitch with Neera in party as she tries and fails to talk about the lich's death, even after Phreya dies. Ctrl+R and Ctrl+Y sometimes fixes this.
Other times, sometimes the dialogue plays as to the nature of the lich's death, sometimes it is skipped entirely and they move on to talking about leaving the tomb, sometimes the order of the conversations swap around and repeat, seemingly depending on their respective positions in the tomb.
Expected: When Phreya and the Lich both die, character should speak about the death of the lich being abnormal, then talk about completing their mission and that they are not leaving.
