On beating Korkorax, the entire party is teleported without the chance to recover equipment, permanently losing items. If Hexxat is dead, this process is instant, and the journal is not updated.
This same behaviour occurs with all of Hexxat's gear when she receives her reward from L, with her equipment being lost.
Typically if the party is removed from a location permanently, such as the Forest of Tethyr at the beginning of TOB, any items on the floor are transported to the player's feet along with them.
