change how the difficulty settings work?
add in more roll factors than just hardcore dnd and very difficult make a legend difficulty to make it more challenging in the new campaigns you plan to put out or even a SCALE difficulty option.

Low Quality Example:
The Idea being that you can go from 50% to 200% on any of the sliders
Easy currently has shorter debuff durations, min player damage and max enemy damage and EZ mode Attacks of Opportunity/friendly fire
Very Difficult has increased enemy damage, full friendly Fire, normal attacks of opportunity, normal player damage and normal debuffs
So sliders could give us something in between, or even an extreme version where all "bad" sliders ae maxed and good ones are minned.
I did miss a box for crits on PCs but it is just a quick example
1. Caster mobs, presuming the AI is improved, scale up rather more steeply than melee types and can suddenly make an area too difficult; and
2. XP Reward increases, which means the character can reach higher levels, which in turn might create further issues.
Basically it is quite a lot of work for every campaign/module/PW creator to re-balance things. That being the case it would have to be a toggle switch, off by default. So the effort might not be worth the candle.

what if instead of the usual difficulty settings we added that players can tweak the different things as they like,
you want an verry easy game but players can die, enemies and party deal full damage or 75% damage, aoo work both ways, firendly fire does 0 or 50 or 70% damage to your allies, go for it, want pertifaction to be permanent or last longer then vanilla, go for it, want charm and domanition to work on you like they do enemies? go for
the choice is yours
i feel that a trully tweakable and customiblze system would be great, players can set it how they like
