How should I prepare, when making a new area?

I'm working on my upcoming mod, and I decided to make a area.

I like this area to have hidden doors on both walls and floor. How is this done, when making the area in gimp/blender?

Do I need a cut out, one with closed door, one with a open.

How is it done in DLTCEP?
