Spell Sequencer Bug.

The spell sequencer in special abilities bar, once used, reappear after rest, altough it is "empty" (it casts no spell, and it disappears until I rest again). And it stacks on numbers. (as in, I have 2 empty spell sequencers now)

In my current save it only bugs if I use skull traps in it...
(the spell is the level 7 one, Spell Sequencer)
Edit: I found a way to fix it. I just loaded another spell sequencer with 3 skull traps and then I saved the game and removed the sequencer (all 3 of them) with EEkeeper. And the thing is now it works just fine, even if I load skull traps in it.
I have 4 spell sequencers that do nothing. Once I use them it disappears, but when I rest again it comes back. If I cast spell sequencer and save 3 spells, sometimes it adds a separate icon that doesn't go on the stack of 4, sometimes it adds it to the stack and increases it to a stack of 5. Only the first usage of spell sequencer in the innate abilities menu works. After it is used once, the rest of them are useless again.
Go ahead and read Gate70's post above and upload/attach any/all of the saves/information he's got there for assistance.
Then @ him and Tresset to see if they can figure out what the heck is going on.
Perhaps if you were to attach a save game file containing the seemingly-broken sequencer we could look into it.
Edit: It appears that the empty extras of this spell are being restored by a rest. No idea how they got there in the first place though. Although... If memory serves I first noticed the empty extras while I was in the underdark in SoA. It would not surprise me if this had something to do with drow form or possibly something else that happens down there.
I ask this because I noticed that when my sequencer has a wild surge it seems to be replaced with a second sequencer upon rest without me actually re-loading and preparing another one. This may actually be a separate issue since the sequencer that appears after rest is fully loaded and will function as expected once used.
"Comet test" is the one from before noticing the issue and "Stark" is from after noticing the issue.
