Whenever my party start complaining about fatigue, I look for somewhere to rest. I go to an inn if there's one nearby, otherwise I camp out wherever I am. Typically, I retreat to sell a big pile of loot, and my party are often fatigued after travelling to the merchant if he's on a different map, so of course they then rest before returning to the fray.
I don't generally find it necessary to add in extra rests specifically to prepare for a battle, but occasionally if "it's obvious" that there's likely to be a major fight through the next door (or whatever) then I'll make preparations including an extra rest. Possibly I'm using spells more sparingly than many other players? I'll buff up and empty my spellbooks on the major set-piece battles, but for routine encounters along the way I usually find that just-charge-right-in-and-kill-them works fine. Sometimes I've found that just-charge-right-in-and-kill-them even works against some boss-enemies ... perhaps because I'm rather completionist, so I tend to be well-equipped and well-levelled-up before I get there. For example, I recently completed a run through the whole saga in which I cast Time Stop (etc.) only three or four times ... not as a deliberate policy, but simply because there were only a few occasions when I needed to bother with an all-out magical assault.
I only rest when everyone is almost dead or when everyone is fatigued. Spells smells...unless I know I'm going into a tough fight. I know...slave driver.
I rest when at least 4 members of my party are fatigued. This can happen due to the Haste spell using, due to long travels between areas or just due to long adventuring in the dungeon.
But when the game shows a big battle is coming (for e.g., before going into the house of Perth the Adept), I rest too in order everyone is prepared.
