Cutscene of Bhaal's Death: How did Bhaal lose an arm?

During the crossing at Boareskyr Bridge, CHARNAME blacks out and has a vision of Bhaal being murdered by the Mad God Cyric.

I have always wondered how Bhaal look like and when I saw the illustration of Bhaal in the cutscene, well, I was quite disappointed tbh. And I have many questions too: the first one being:

1. How come he lost an arm? Lost to whom or what?
2. What is Bhaal's preferred weapon of choice? Or does he fight with claws/talons in one of his avatar's form?

    @PaulaMigrate Thank you. And in the cutscene, is that one of Bhaal's avatar forms? I read that he has three: "a shape-shifting one called Kazgoroth, a human-sized one called the Slayer, and a giant called the Ravager" It can't be the Slayer or Ravager, so I guess that must be Kazgoroth?

    When I was in school if you annoyed my gym teacher (something that was surprisingly easy to do) he would threaten to "rip your arm off and beat you to death with the soggy end". Maybe that's what Cyric did to Bhaal?
