Totemic Druid is very useful only if U start from scratch on HoF difficulty (cuz of Summon Spirit Animal available starting from level 1). Otherwise, U can take a Sorcerer instead. 😎
At which level do you suggest starting heart of fury with a 4 man party?

Continue to play on Insane, it's already quite challenging. Some parts of the game are uber hard on HoF even with level 30 chars (and U can't switch from HoF to easier modes). ☠️
P.S. If U still wanna play and survive on HoF, I'd suggest a party of level 15+ (and of level 30 ⚡ in some parts like Trials of the Luremaster). 😎
Hmm... I don't remember now, there is a real problem with mage scrolls in IWD. You can go with Hjollder from Kuldahar to Lonelywood (the Heart of Winter extension), buy magic scrolls there and come back. However, in this case, you should somehow complete the very difficult Burial Isle. 💀 On the other hand, you will get tons of non-combat XP, while completing quests in Lonelywood and Barbarian Camp. 😎
Bag of Holding and all mage scrolls. If you go into the Vale of Shadows, you need also a blunt weapon vs skeletal undead and a magical (at least, +1) weapon vs Greater Shadows, Mummies etc. 👻

Dwarven defender Main tank, axes and hammers later dual wield

DD weapons: Axe and Hammer (up to High Mastery). There are throwing axes in the game. 🪓
F/M/C weapons: Mace and/or Flail + Sling. 😎
F/M/C, Dwarven Defender and Dragon Disciple (Sorcerer's Kit) looks fine to me. 😎

Hmm, I'd suggest weapon+shield for F/M/C and Dwarven Defender. There are good shields in the game. 🛡️

The most useful divine spells 🧚:
Level 1: Armor of Faith, Cure of Light Wounds, Entangle, Sanctuary.
Level 2: Barkskin, Charm Person or Mammal, Draw upon Holy Might, Hold Person, Find Traps.
Level 3: Animate Dead, Call Lightning (outside), Cure Medium Wounds, Summon Insects, Glyph of Warding, Holy Smite / Unholy Blight.
