It is painful but like every pain it dependents on the woman and the childbirth that take place how much it will hurt
Typically I'll go a 60/40ish split in favor of the dudes doing at least 1 Good/Evil/Neutral playthrough for both genders and then have a self-named character (where that extra 10% split comes in) so I can play from the point of (If this was me...what would I do?) I mean, whats the point of having all these in-game dialogue variables if you can't even see:

Yeah, adoption always seemed like the way to go to me. None of the pain, plus you can give some poor orphan kid a decent home. And it doesn't matter what parts you or your partner of choice have, or even if you have a partner (although I hear raising a kid by yourself is pretty hard).

of course we have a choice, I think the pain is worth it and so all the woman that choose to give birth, pain is something that pass after a time it not forever We also have a choice if to take Epidural (the drag which given for painkiller at birth time) or not so now days it easier

I will typically play both genders and several races if I like the game enough for multiple plays, but the first time if given the option is usually as a female character.
I get kind of tired of generic dude warrior guy presented as the standard in most games when there is no choice given, and I certainly don't see the character as an avatar of myself in the game, I see them similar to the way I see a character in a book but where I get to have influence on their decisions and the outcomes.
