    @Bubb has it! Kahrk says he’s pleased with you when your free him, but he tries to kill you anyways. He caused the previous adventuring party to go insane and then they... killed each other, I think. He calls himself the greatest ogre mage, and at level 12 I think he’s right.

    Well I was going to say Sarevok, but everyone knows he came back so it can't be him. Therefore my second guess is Alorgoth.

    His rage is so great it nearly tears apart that Shadow Realm you are in, although the same could be said for Rasaad and he doesn't die no matter what you do.

    In the end, Charname is unaware of what may have happened to Alorgoth, but in Rasaad's epilogue it confirms he is still alive and kicking.

    maybe also the demon prince in WK can fit the requirements.
    his rage for being imprisoned in the keep is great, if you kill him you only send him back to his plane of existence and you don't meet him again after the fight so you never know it.
    the 3rd requirement is barely fulfilled, as you are reported of the fact that you don't really kill him, but the italics "you" possibly can mean that you don't have a direct experience of him being still alive.

    Elminster? You can "kill" him twice, but he comes back. If you fail, his cast strong spells before teleporting away.

    Maybe my choice was outside the bounds of this thread; I probably haven't given enough clues to hint at the character. So, I'll give this one 4th-wall breaking hint that hopefully speeds us along:

    5. I am from Icewind Dale (the game), and I am located deep, deep underground.

    @smyth25: We have a winner!

    1. Malavon has a spell he made himself called "Malavon's Rage"
    2. The first time you fight him, you kill a lowly simulacrum.
    3. The second time you fight him, you again kill a simulacrum, but you never know this. This detail is revealed in Icewind Dale 2.
    4. The cloak Malavon is wearing has the description: "It is traditionally worn by the most talented practitioner of divination magic in Rilauven's academy of sorcery."
    5. His character is found in Icewind Dale, and can be found in Lower Dorn's Deep.

    2. His great scars are his missing eyes
    3. He does not leave the lich grave area until the unseeing eye has been put to rest (killed).

    The Unseeing Eye

    1. To get to him you literally need to jump into the Pit of the Faithless
    2. To kill him you need the two parts of the Rod
    3. And "See What I Mean" is just a pun because The Unseeing Eye cannot, although the game devs forgot to edit his sprite to show that he had no eyes.
