My biggest moment of shame related to BG was this. Way back in the days before it was released, I went to GenCon in Wisconsin. On the floor they were showing this Demo for a new D&D game that was coming out for the PC. I stood in line just to get to play it. I get my 10 minutes on the game itself and say "It's real time?? I'm not going to like this whole Baldur's Gate thing. I might just give it a pass when it comes out."
One time, Jaheira and another party member (forget who) died, somewhere in the Underdark. I had Anomen with a raise Dead memorized, so I brought back Jaheira with 1 HP. Then I attempted to have her cast Harper's Call on the other dead NPC. What? Why did she just die again? I had completely forgotten that Harper's Call damages the caster...

Seeing if I could resolve the Hull's antidote quest by putting the cow out of its misery.
(Surely someone else has done that just to SEE what would happen...)
Believing I had finished the game when I met Gorion again under Candlekeep (I knew a mighty mage couldn't just be killed by a tin pot knight in armour! Good to see you again Dad!)
Not finding Liera's book of unknowing... But maybe I DID!
I made plenty of fails in BG2 but they just don't seem that epic... I killed a whiny ogre in a circus tent once... didn't feel like I was missing anything...

I don't get all the hate for NWN. it wasn't Baldur's Gate, full stop. But then it really wasn't intended to be. It's focus was always on online and the ability to create your own adventures. In that it did spectacular as evidenced by the fact that there are literally thousands of quality modules (and more of lesser quality) and the game is still played today.
Yes, the camera was junk. Yes it was 3E(ish) and yes it wasn't as pretty as the custom static images of BG, but then anyone could make modules in it and tell any story they wanted. In that I think it was quite a success. It just wasn't Baldur's gate.
Back on topic, another epic fail I did was, after traipsing around the wilderness of BG2, I encountered these gnolls and ogres. with Keldorn at my side, we handily defeated..... the..... Knights???? Instead of hunting down the dastardly villain who set me up, we went back to the City for some much needed rest, only to get caught up in a quest associated with Jan (or was it Planar sphere? I can't remember). Anyway, eventually we got the message that our reputation was ruined and Keldorn fell.
I played BG2 first before BG1, and I didn't know much about D&D either then. Sorcerer sounded like the coolest class. There was this spell with a cute icon of an animal that I totally must have so I chose Find Familiar as one of my spells.
I also used Nalia for thieving instead of Yoshimo, and couldn't understand why she couldn't pick all the locks.
@meagloth, your comment made me smile, but I assure you that high level Anomen is capable of blowing up greater mummies, vampires, and as rumour has it even liches

Yea its very much a "crap I have no other means of reviving this dead party member" kind of spell. Best used when all other options are unavailable.
it also seems to randomly not work sometimes. jaheira takes damage, but the dead party member stays dead. so i never use it
Thats because it lowers the revived party members constitution by 5. Unfortunately at the moment that is causing the party member to lose health from the constitution loss after they've been revived, which kills them. Hopefully the next patch should address this (which I believe is just going to make it so that all stats other than constitution are reduced).
But in terms of dumbest things I've ever done once when playing Tutu I had my wild mage cast a buff spell while in Beregost. Of course the result of this was to get a wild surge and spawn a demon.
I did this right near the entrance to the Red Sheaf Inn (after Silke's quest). Brave, brave sir garrick didn't get a chance to run away either. I did though. His sacrifice will always be remembered

@Blackraven@meagloth - I remember way back, seeing screenshots from a guy who'd soloed BG2 with a Priest of Lathander - he blew up even Kangaxx the Demi-Lich with turn undead...
IIRC, he was playing with Tactics and Ascension on insane difficulty so he was around level 40 Cleric at the time - he hit the ToB XP limit well before the end of SoA. :-)
Back on topic, did you know that if you summon a Planetar or a Deva and send it into battle, then use AoE spells which also damage your summons, your own summons will turn on you? No? Neither did I... Until that day in Sendai's enclave when a p***ed-off Planetar came back at me, hair on fire, sword unsheathed and with a red ring around its feet!
Travelling to Mutamin´s basilisk & stoned gallery without a potion of gaze reflection, or a spell/scroll of protection from petrification, or to pirate shores, wherein charming sirens take me in mind control, because I don´t have the Greenstone Amulet, or Potion of Clarity... what else there is to encounter, with without the proper buffs is rather not recommended? Oh, the demon knight under Durlag´s tower, and Aec´Letec - trying to match them without a scroll of magic protection is such a waste of time. Oh, and the occasional ghoul/ghast/demon/lich gets me by stunning someone in the party, or making some members of the group panic... ah, and since I´ve dowloaded bg2:ee, I haven´t experienced it yet, but I do expect facing vampires to be such a chore and bother of either protection from negative energy or restoration...
Once, when I was very new to BG, I got trapped in Nashkel mines. After killing Mulahey I quit the game and return to it after several days. From my first playthrough I remember that there is a shortcut to the surface, but I though that the way leads from Mulahey's cave. I spent hours in that tiny cave looking for hidden door or something I missed. Eventually I decided to go back to the mine entrance and as soon as I stepped from the cave I remembered, that *this* was the area with the shortcut. Man, I felt stupid that day.
On a BG:EE Sorcerer solo run, I've ran out of Potion of Healing, so I used the Wand of Frost with The Belt of the Antipode and the Boots of The North equipped, very silly indeed, but useful.
In BG2:SoA, stealing from Ribal thinking that the Amnish Guard was one of those pansies that wake you when you sleep.
In PS:T, killing everyone in the Mortuary for XP, when I realized all the things I had missed (when playing it for the second time) I decided to play it again entirely.
I had heard a rumor in the beginning that Xzar and Monty actually left the group just prior to the final battle if you had them with you. So I always made sure that I dumped them (if I took them at all) far enough back that I could bring some new party members up to speed for the final battle. turns out it was only ever an urban myth.

sure enough I went back into the game only to discover I had turned my character into a cow
