Sorry, everyone but you missed a few critical steps.

To unlock the Gazebo Dungeon you need to complete SoD on Legacy of Bhaal difficulty, export your character then complete the game again on Story Mode with the same character and lastly talk to the squirrel in the last area.

However there are two squirrels. If you talk to the wrong one you have to repeat the whole process.
