If you keep Joia's flamedance ring from the Friendly Arm quest to the end of the game instead of giving it back to her, you can find it in Irenicus' dungeon at the start of BG2. Hang on to it and to Captain Dennis' gong from the limited wish quest. With these two items and a modest sum of 7,500 GP, Cespenar can create the Gong of Might which can be used twice a day to render all enemies unconscious from the stink of cow dung (save vs. smell to resist effects).
Shandalar and Thalantyr are old buddies from wizarding school.
Ravenclaw, obviously.
If you don't go to the Gnoll fortress until after Candlekeep, you'll find Dynaheir has become Queen of the Gnolls and is preparing to march on Nashkel.
You didn't actually save Rielev in Chateau Irenicus. Rielev's unit has a backup energy source: 5 minutes after you leave his room, he wakes up again, and is in horrible pain just like before.
But if you have 21 Intelligence in ToB (a Potion of Genius works fine), you can go back to Rielev's room via a special dialogue option in the Pocket Plane, and finally release him from his suffering by shutting down his unit.
Or, if you're a wizard or a thief with the Alchemy HLA, you have another option instead: you can transport one of the preserved bodies from the tanks in the other room, and Rielev the Pickle will be able to spend the rest of eternity with his soulmate: a sexy lady pickle named Cucumbria.
When Jan maximizes his thieving skills in ToB he will be able to use his pick pocket ability to pick your pocket plane. This will unlock a new side quest where he will use the pocket plane to smuggle fine merchandise into the Abyss. A concatenation of catastrophic events will culminate in a tough battle with a new breed of monkey/turnip tanar'ri.

There is an esoteric 'rule of thirds' in IWD. If you always choose the third dialog option for the first two chapters, you will be granted the ability to choose all dialog options for the entire game. Role-players should pay especial attention to this.
If anyone paid attention to the ctrl-num5 trick for BG1, a similar option exists in BG2. It allows you to start the game as Irenicus, battling to kill a bloodthirsty Bhaalspawn before he is able to ascend to godhood. It is especially interesting as Dynaheir and Khalid are reanimated and may be recruited, and even romanced, by Irenicus.
Hexxat originally had actual character development planned. But they ran out of time and had to release the dungeons, so they slapped a character together.

Does that mean modders are working on an Unfinished Hexxat Business pack to finish her character development??
When you have pickpocketed each and every guard in Athkatla, Aran Linvail would appear and give you Ring of Amnish Luck, granting +5% to all your thieving skills.
If you have melicamp in your quick slots he will absorb one petrification attack made against you. After this however he will disappear from your inventory.
The Vampiric Sword and a Ring of Wizardry can be forged together to create the Sword of Dying Magic. This cursed sword drains spell slots from its wielder and gives them to the creature being attacked. When it has no more spell slots to take it resorts back to draining health.

You can find Montaron's body in a chest in the Harper Hold after Xzar dies, but it's not actually Montaron's body. It's actually a polymorphed version of Colette. The real Montaron was Renfeld, and the mage that Colette killed was actually Edwin, under orders from Thay, posing as Xzar to infiltrate the Zhentarim.
The Edwin you meet in Mae'Var's guild hall is the real Xzar, which is why Edwin has a slightly different portrait in BG2. The real Xzar is the one who turned Montaron into Renfeld, in an attempt to infiltrate the Harpers, but the fake Xzar (Edwin) didn't know that, which is why he asks you to find Montaron. The real Xzar later turned Colette into Montaron's body to avoid attracting your suspicion if you tried to investigate later.
It's actually an Easter Egg, referencing the fact that Carbos is actually a polymorphed Shank, and vice versa.
@semiticgod Actually it was the @pegasusprince, I also went by the name @swiftwind, but @She-ra said we could never be together because I was a Pegasus and she would only date dragons. Now I'm a Dragon with @she-ra, @stargirl, @powergirl, and a few other blondes who keep acting like they're my wives.
@semiticgod "Was" is the keyword in my sentence. What happens after the Dragon awoke, I cannot be blamed for; I was in my den being fed grapes by @Dagger and @blackcanary. While @terra, @Magik, and @starlight fanned me with leaves from a palm tree...

(But how come they both were part of the old game when we didn't have Neera?)
The romance with Hexxat is actually a Necromance.
